Dual Diagnosis/Young Adult

Pay Scale: $22.00 and up

$1500.00 SIGN ON BONUS

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Case Manager to join our talented multi-disciplinary team in our Community Support Program working with adults living with severe and persistent mental illness.

As a Riverbend Case Manager, your primary responsibilities will be working with clients to provide clinical consultation, case management and therapy services using a number of evidence-based models. You will work closely with treatment teams within Riverbend as well as outside agencies. This team serves primarily young adults and/or individuals with intellectual disabilities. This is a great opportunity to work on the front lines of community-based mental health services, and help our clients achieve optimal recovery and independence.

Essential Duties and Responsibilities:

  • Provide case management and assist with inter-agency coordination
  • Provide crisis management
  • Provide functional support services
  • Work collaboratively with client and treatment team to plan for treatment
  • Document clinical services in accordance with medical necessity guidelines and agency standards

Education and/or Experience Requirements:

  • Bachelor's degree in Psychology, Social Work, or related field required; Master's degree preferred.
  • One year of work experience in a human service or mental health field is preferred.
  • Valid driver’s license, personal vehicle, and auto insurance is required.
