Full Job Description

Center for Human Development, Inc. is seeking a part-time (36 hours weekly) Family Advocate to work independently under the guidance of the Public Health Babies First Nurse and receives clinical supervision best suited to meet the family’s needs. The Family Advocate visits prenatal moms, children 0 through 4 years and children with disabilities 1-21 years of age. The Family Advocate works on an ongoing basis with families identified as “at risk”. This position partners with the family in improving their use of resources; parenting, coping and problem-solving skills and encourages healthy growth and development by reducing family stress and meeting needs of parents.

 


The Requirements:

 


  • Bachelor’s Degree in Human Services or related field: or combination of three years education, and experience in public health, mental health, alcohol and drug, health occupations, or health education.
  • Ability to develop and implement individual and group skills training programs.
  • Ability to work effectively with a team, other professionals, and children and families.
  • Ability to organize and facilitate groups.
  • Access to a vehicle, valid Oregon driver’s license and proof of insurance.
  • Must pass all criminal history check requirements and drug screenings.

 

The Essential Functions:

  • Provide primary case management and act as an advocate for the family with other community agencies.
  • Work independently to establish a case plan with goals, objectives and activities to meet family needs. Meet with Babies First, Maternity Case Management and Ca Coon nurse on a regular basis to evaluate family status regarding TCM.
  • Identify at–risk/over-burdened families and have experience with issues faced by pregnant and parenting individuals and families.
  • Establish and maintain a trusting relationship with at-risk families by providing regular contact through home visits.
  • Continue to provide long-term service after intervention services from outside agencies have been in the home (IHBS, SCF, ESD, etc.).
  • Apply working knowledge of parent-child interaction, child growth development and dynamics of child abuse/neglect to promote positive dependency needs of parents.
  • Assist parents in making and attending appointments with doctors, social service agencies, etc. Work to decrease family isolation and dependence through referral and support in engaging in Babies First activities and other community resources and activities.
  • Perform developmental assessments.
  • Provide crisis intervention/management.
  • Assist families with paperwork needed in application for resources such as medical, housing or financial assistance.
  • Provide data collection and evaluations for program tracking to state and county.
  • Collaborate with other agencies to assist participants in reaching full potential.
  • Work effectively with diverse families.

  • Work with WIC coordinator to assist families with in home certifications, which include taking height, weight and checking iron; according to WIC state regulations
