Salary Range: $49,000 - $51,000

 

Schedule: First (Full-Time)

  • Monday-Friday: 8:00 a.m.- 4:00 p.m.
  • Occasional evening and weekend work may be required as job duties demand.

 

What You Will be Doing:

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Initially and on an ongoing basis, instructs and advises the PDP parent and individual(s) served (and the prospective PDP parent and future individual(s) served) regarding PDP requirements and practices for participation in the program and continued good stewardship of the program.
  • Enforces policies and procedures that address PDP management practices in all areas – from confidentiality and compliance to training and internal controls, among others.
  • Assists the Director and Administrator in the representation of Boundless in any audits or provider compliance reviews conducted by staff or representatives of the Ohio Department of Job and Family Services, the Ohio Department of Medicaid, the Ohio Department of Developmental Disabilities, or specified County Board of Developmental Disabilities.
  • Communicate daily with new family referral sources using independent professional judgment to assist them in identifying if PDP services would be appropriate for the family and assisting individuals with selecting services that will meet their needs and onboarding onto the program accordingly.
  • Assists the Case Managers in communication with County Boards regarding new families, current families, medication administration requirements, and other case-related information.
  • Update and maintain the Advisor database with current employee and family relationships, demographic information, notes, and attachments.
  • Monitors and ensures all persons served that require nursing delegation have the appropriate training and documentation maintained in the person served record. Works with delegation agency to maintain compliance and home visits as required.
  • Monitors and ensures all staff requiring Stand Alone certifications and Medication Certifications are up to date and in compliance and completing MARs as required. Follow MUI and UI reporting guidelines per DODD and I am Boundless rules and regulations if necessary.
  • Ensure service delivery documentation for persons served is reviewed for accuracy and completeness. Provides training and support to staff and families when needed on the Anywhere system to achieve accuracy.
  • Cross-trained with PDP Fiscal Specialist for all job duties and responsibilities.
  • Any other duties assigned by the supervisor.

 

Required Qualifications & Experience:

  • High School Diploma or equivalent.
  • 3-5 years of customer service experience.
  • Excellent computer skills – including spreadsheets and word processing tools – and the ability to learn new programs expeditiously.

 

Preferred Qualifications:

  • Experience with Advisor and Anywhere software.
