As part of a multi-disciplinary treatment team, the Case Manager provides services and supports to help clients function effectively as they work toward behavioral health recovery. The Case Manager helps individuals to obtain needed goods and services in their community, provides life-skills education and support as well as counseling interventions to reduce behavioral health symptoms. The Case Manager is guided by strengths-based Recovery Principles which support client autonomy and independence.

CORE RESPONSIBILITIES:

  • Provides psychoeducation and skills training to clients individually or in group setting
  • Provides supportive counseling and other interventions which support therapeutic goals established by the Clinician
  • Completes initial Brief Screen for clients seeking services at Centennial
  • Completes Case Management Assessments
  • Develops and monitors individualized case management plans
  • Assists clients in obtaining needed goods, services and benefits both within the agency and in the community
  • Oversees a caseload of clients receiving Medication-Only level of care; completing required documentation and coordinating psychiatric services with medical team
  • Collaborates with other Centennial service providers to engage clients in care
  • Facilitates team-based care meetings with other Centennial providers and community collaterals
  • Provides services in a variety of community settings, including client homes
  • Maintains accurate and timely clinical records consistent with Center standards
  • Attends required agency and community meetings
  • Participates in regular supervision sessions
  • Performs other job duties as assigned
  • Demonstrates a commitment to continuous skill development to competently serve individuals from diverse cultures and backgrounds.

EDUCATION:

Bachelor’s degree in psychology, social work, or other human services field

SKILLS, KNOWLEDGE, AND ABILITIES:

  • Ability to work well with clients presenting with complex needs
  • Able to identify and nurture strengths and support client rights to make independent decisions
  • Skills to serve culturally diverse populations
  • Effective written and verbal communication skills
  • Demonstrates effective time management and the ability to multi-task
  • Ability to work as a team member
  • Knowledge of basic computer literacy such as e-mail communication and word processing
  • Ability to travel within the Center’s service area or to other locations as needed

Job Type: Full-time

Pay: $20.67 - $24.78 per hour
